SMA-ADA Dial-A-Ride (DAR) - How to Apply
To be ADA Dial-a-Ride certified, passengers must complete the following steps. Once the steps have been completed and all information is submitted, you will be notified within
twenty-one (21) days whether or not you have been ADA Dial-a-Ride certified.
Step 1
Request Application To request an application for ADA Dial-A-Ride service a person may:

Step 2
Completely fill out Application Form Incomplete forms will delay the review process. If the application is incomplete it will be returned to the applicant for completion. If you have any questions regarding application forms, call the RTD administrative office at 209 982
4514, extension 8432, and ask for assistance.
Step 3
Submit Application Form to medical professional Take the form to a medical professional who can best diagnose your disability and assess your need for ADA Dial-a-Ride service. The doctor will need to review the
vertification criteria and Application before completing the
Vertification Form.

Step 4
Return Application/Certification Form Forms may be mailed
to San Joaquin RTD at P.O. Box 201010, Stockton, CA 95201, or delivered to RTD’s Administrative Office at
421 East Weber Avenue, Stockton, CA 95202.
Step 5
Notification of qualification RTD staff will carefully review each application to ensure that only qualified persons are approved. Upon
completion of review, a notification of certification or an explanation of denial will be mailed. If your Application for Dial-A-Ride service was denied, you have the right to appeal this decision.
